Our office hours are Monday-Friday 8:00 AM to 4:30 PM. Please call during business hours for an appointment: 456-8191. You can request appointments also using our online forms for appointments in Fairbanks or Delta Junction.
If you cannot make your appointment please give 24 hours notice if possible. For urgent appointment requests, please call the office.
We accept most major insurance carriers, including Medicaid and Medicare. We are Blue Cross preferred providers.
Once you have made your appointment, you can click on the links below to download the New Patient Forms. You may print and complete the patient forms prior to your arrival. This will help to speed up the new patient registration process.
